GAA Check Request Form - Editable Word Document/1Page. Sport Clubs use this form to request funds from GAA.  This form must be signed by the club treasurer and amounts over $500 require a coaches signature.

Deposit Form

GAA Deposit Form - Editable Word Document/2 Pages.  This form is used by GAA Sport Clubs that need to submit monies to the GAA treasurer.  The submitter must count and document all checks and cash and another person must count and verify all the financial information.  Submitter must contact Cynthia Stastny, Treasurer, to arrange deposit of funds.

Fulton County Fundraising Application 

FCBOE Fundraising Application - PDF Document/1 Page.  This form is to be used by any GAA Sport Club that wishes to do fundraising.  Applications must be signed by the team coach and submitted in triplicate.  Any application involving the sale of spirit wear must have mock-ups attached to the application and approval from Tanya Deal, GAA Spirit Wear Manager. 

IRS 501(c)3 Non Profit Certification Letter

IRS Certification Letter - PDF Document/1 Page.  This document may be used by GAA Sport Clubs that need to provide our non-profit certification letter to sponsors or vendors.

W-9 Form

IRS W-9 Form - PDF Document/1 Page.  This form may be used by GAA Sport Clubs that need to provide tax ID information to sponsors or vendors.  This document is signed by GAA treasurer, Cynthia Stastny.
